Three sheets from one list
Every sheet starts with the same header: name, date of birth, allergies, emergency contact, and doctor or pharmacy. Empty fields print as blank lines you can fill in by hand.

Printing on A4 or US Letter
When you print, only the sheet comes out; the form and the rest of this page are left
off. The margins work on both A4 and US Letter. The list and the chart
have a lot of columns, so landscape usually fits better. To keep a digital copy, choose
Save as PDF in the same print dialog.
The same list on your iPhone
A printed list is mostly for other people, such as a pharmacist or a nurse on
admission. For your own day, Doz keeps the
same medications and dose times on your iPhone and reminds you at each one. It groups
medicines under the prescription they came from and counts each one down as you log
doses. It needs no account, keeps your data on your iPhone and in your own iCloud, and
is free for up to four medications.

Common questions
What should a medication list include?
The name of each medicine, its strength, how much you take per dose, when you take it, whether it goes with food, what it is for and who prescribed it. Your name, date of birth, allergies and an emergency contact go at the top. This tool prints all of those.
Is this medication list template free?
Yes. There is no charge and no sign-up. The list is built and printed in your browser, and nothing you type is sent anywhere.
Can I save the list as a PDF?
Yes. Press Print and choose Save as PDF as the destination in your browser's print dialog. Download CSV saves the list as a file you can open in a spreadsheet.
Can I print a blank medication list template to fill in by hand?
Yes. Choose how many empty rows you want, from 1 to 40, and press Build a blank template. It prints the same header and columns as the filled list, with empty rows to write in.
How do I use the schedule chart as a medication log?
Each time of day you ticked for a medicine gets seven boxes, one per day from day 1 to day 7, left to right. Tick a box when you take that dose. A medicine with no time ticked goes under As needed / other, so nothing is left off.
Does the page save what I type?
No. Nothing is stored in your browser and nothing is sent anywhere, so the form is empty again when you reload or close the tab. Print the list or download the CSV first if you want a copy.
What this tool does not do
This tool is for planning reminders, not medical advice. It lays out what you type; it
does not check whether a medicine, a dose, a time or a combination is right for you.
